Overview
Cagrilintide is a synthetic, acylated long-acting analog of amylin (islet amyloid polypeptide), the peptide hormone co-secreted with insulin from pancreatic beta cells. A fatty-acid modification confers albumin binding and an extended half-life relative to native amylin, while sequence substitutions improve solubility and stability. It is studied as a dual amylin- and calcitonin-receptor agonist reference compound, and has been characterized in combination with the GLP-1 analog semaglutide.

The research literature attributes its activity to agonism at the amylin receptors (calcitonin-receptor cores complexed with receptor-activity-modifying proteins) and the calcitonin receptor.
